    Includes single vision, bifocal, trifocal and lenticular lenses. Certain options (blended lenses, scratch-resistant coating) may cost extra.

    Plan pays 100% (after $20 lens/frame copay)

    Plan reimburses as follows: Single Vision: Up to $30 Bifocal: Up to $50 Trifocal: Up to $65 Lenticular: Up to $100

    Plan allowance up to $130, plus 20% discount off balance
